Quality Initiatives Transition of Care

We aim to maintain a comprehensive quality management program to objectively monitor and systematically evaluate the care and services provided by our hospitalists.

The program is designed to provide on-going, proactive evaluation of compliance with performance expectations of the contracted health plans.

• Transition of Care ensures that the patient’s medical care continues uninterrupted and improves coordination across the continuum of care.
• Promotes seamless transition from the Hospital to home, Skilled Nursing Care or Health Care.
• Reduces unnecessary readmissions that may increase risk, harm and cost to patients.
• Addresses issues in medication management, post-discharge follow up and plans of care for patients who move across health settings.
• Promotes provider accountability and active patient/family engagement.
